Providence Health & Services Client Coordinator in Chewelah, Washington
Providence is calling a Client Services Coordinator (Home Health Services, Days, 0.0 FTE / on-call) to Providence DominiCare in Chewelah, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Accept referrals, completes intake of client information and makes initial home visit.
Assess client health and functional abilities and collaboratively develops service plan to assist client and family to meet the client's mutually identified needs.
Ensure that appropriate care is provided to clients by directing and coordinating the activities of the care giving staff.
Assist with caregiver orientation.
Identify high risk clients, collaborates with Caregiver Coordinator in planning for in-home orientation of caregivers to their equipment and special needs.
Responsible for orienting & training caregivers to reduce risk of injury to self & clients
Report to Caregiver Coordinator re: staffing needs, scheduling changes & all service concerns.
Required qualifications for this position include:
Academic degree Or equivalent educ/experience
Maintains current Certified Nursing Assistant and/or Caregiver Certifications.
